- the title of the supervisor for the position
- the number of employees the H-1B worker will
supervise, if any
- a statement of any licensing requirements
for the position
- the proposed salary
- the employer's Federal ID Number
- location where employee will work

The following filing
fees should be paid by the employer.

- check in the amount of $320, payable to the
US Citizenship and Immigration Services

check in the amount of $1500,
payable to the US Citizenship and Immigration
Services (CIS added a Training Filing Fee
effective March 8, 2005). If your organization
employs under 25 employees the Training Filing
Fee is $750.00. If your company is an H-1B Cap
exempt organization (such as a college) this fee
does not apply.

check in the amount of $500,
payable to the US Citizenship and Immigration
Services (CIS added a Fraud Fee effective March
8, 2005).

check in the amount of $1000,
payable to the US Citizenship and Immigration
Services should you wish to use CIS’s expedited
processing option. Premium Processing reduces
the adjudication time of a case down to 15 days
or less.
